Staff of the Department of Anesthesiology and Intensive Care at the NMAPE named after PL Shupyk actively participate in the educational and practical activity of the clinical bases of the department.
One of such bases of the department is State Institution “Heart Institute Ministry of Health of Ukraine", in which a large number of operations on the heart with cardiopulmonary bypass are performed daily. However, despite the large number of complex surgical interventions, the "Heart Institute" has unique operations that require high surgical and anesthesiology skills.
One of the pathologies undergoing surgery at the institute is Ebstein's anomaly. According to the literature, Ebstein's anomaly is a very rare pathology. It occurs in about 1 case in 20,000 newborns and is 40% of all congenital heart defects with a lesion of the right atrioventricular valve. According to D. Roberson and N. Silverman, infant mortality with Ebstein anomaly reaches 80%, and in fetuses 90%. Of those who survived the first 3-6 months of life, ˗ 70% live to the age of 2, and only 50% live to the age of 13. Only 34% of patients survive to age 20.
The radical surgical correction of Ebstein's anomaly was once again successfully performed by the international surgical team, which included specialists of the State Institution “Heart Institute Ministry of Health of Ukraine”, Professor Todorov B.M. and Professor Kalangos A. (Greece). An 18-year-old patient underwent an anatomical correction of the tricuspid valve, which resulted in the valve being remodeled and corrected intracardiac hemodynamics. Anesthesiologic support provided by Professor Loskutov O.A. consisted of continuous monitoring and correction of hemodynamic disorders, blood gas composition, acid-base status, etc.
Due to the extensive experience and high professionalism in the treatment of congenital heart defects, the coordinated work of the surgical, anesthesiologic and perfusion teams, the patient was successfully operated and transferred to spontaneous breathing after two hours.
The further course of the postoperative period went without specifics.
This type of surgery and the technique used for surgical correction and anesthesiology is new and progressive. In the future it is planned to expand the range of such operations in Ukraine.
