A unique cesarean section was performed on the basis of the Department of Obstetrics and Gynecology No. 1 of the Shupyk National Healthcare University of Ukraine in the Vyshgorod Central District Hospital in an obese patient whose body mass index exceeded 45. Due to a significant layer of subcutaneous fat and difficult access to the lower segment of the uterus caused by a pronounced panniculus (apron-shaped fat deposits), it was decided to perform a supraumbilical incision with subsequent fundal cesarean section. This was also due to the presence of a previous cesarean section in the anamnesis. In the second minute of the operation, a healthy full-term girl was removed, weighing 3520 g, 52 cm tall, who received an Apgar score of 9/10 points. Thanks to carefully planned tactics, the operation was carried out with minimal blood loss, lasted about 35 minutes, and the postoperative period for the patient passed without complications, with minimal pain and without the need for additional administration of painkillers.

The happy mother, together with her newborn child, was discharged home on the fifth day after the operation.

The operation was performed under the guidance of Professor O. Golyanovsky with the participation of Doctor I. Desikevych and postgraduate student of the department S. Frolov.
